Homeopathy for Gout: Colchicine, Not Colchicum 30C
Homeopathic medicine for gout is chosen on the attack picture — Colchicum when the great toe is red, swollen and cannot be touched, Ledum when it is cooler and better from ice — not as flare therapy. Gout is urate crystals. First-line for an attack is colchicine, an NSAID, or a glucocorticoid, started as soon as the joint lights up. Urate-lowering (usually allopurinol) is for frequent flares, tophi or damage, treat-to-target <6 mg/dL. A 30C is not 1.2 mg colchicine. A hot joint can also be septic — that is 108, not Benzoic acid.

What gout is (and is not)
(standard clinical picture): sudden, often night, intense joint pain — classically the first MTP — with swelling and extreme tenderness. Untreated attacks last days; repeats damage joints and grow tophi.
Men are more often affected before menopause; risk rises with kidney disease, diuretics, beer, and obesity. It is not “all heel pain” — see heel pain.
What actually has evidence (do this first)
ACR 2020 strong recommendations:
- Flare: colchicine, NSAID, or steroid (oral, IM, or intra-articular) as appropriate.
- Low-dose colchicine for an attack is often 1.2 mg then 0.6 mg after one hour — kidney, liver and drug interactions (including some antibiotics) matter.
- ULT: allopurinol first-line, including many with CKD; start ≤100 mg (lower in CKD); titrate to SU <6 mg/dL.
- When starting ULT: anti-inflammatory prophylaxis 3–6 months.
- Do not stop ULT in a flare.
Diet is adjunct: less beer, spirits, sugar-sweetened drinks, organ meats. It does not replace indicated allopurinol.

Never self-dose autumn-crocus Q. The plant is toxic. 30C has no colchicine if made properly — which is why it cannot stand in for the tablet.
